The W-4 form lets you determine how much tax is taken from each paycheck. Because your tax situation may change, you may want to refigure your withholding each year. You can now make changes to your W-4 form online via my.pitt.edu. You may also complete the bottom section of the paper W-4 form (including #5, the number of allowances you wish to claim) and return the form to the Payroll Office by the 15th of the month. A worksheet is provided at the top of the form, as well as, information on the reverse side to help you determine how many allowances to claim.
Employees who make less than $12,000 per year or who have another job in Bradford Township which withholds the local services tax from their wages, may be exempted from having this tax withheld from their pay. To be exempted from the tax, an employee must complete the Local Services Tax Exemption form each calendar year.
Employees who wish to change their address may now complete the Residency Certification form online via my.pitt.edu. A paper Residency Certification form may also be obtained from the Payroll Office. W-2s are sent to the address on file as of the January monthly payroll deadline.
To change your bank information for direct deposit, the Direct Deposit form must be completed and returned to the Payroll Office by the monthly payroll deadline.
Part-time temporary employees are to complete the Department Timesheet. This timesheet is used for part-time temporary staff employees and Non-FWS student workers. The form must contain an account number and the signature of the account administrator. Hours worked should be entered with times in and out each day and the total hours per day.
