Forms and Information

W-4 form  - this is a federal income tax form which lets you determine how much tax is taken from each paycheck. The tax withheld will show on your pay stub as “Federal In” for Federal Income Tax. You will need to complete all the highlighted areas of the form, which includes the number of allowances you claim. A worksheet is provided at the top of the form and information on the reverse side to help you determine how many allowances to claim.

Because your tax situation may change, you may want to refigure your withholding each year. If you find that you need to change your W-4 form information at a later date, contact the Payroll Office at 814-362-0251 for a form or print one from the link provided above or print one from the www.irs.gov web site. When you complete that form and return it to Payroll, the information will be processed in your next paycheck. 

IF YOUR ADDRESS CHANGES, you will need to change your address with the Payroll Office both to help the university comply with the tax laws and to ensure that you receive important mailings such as your W-2 Forms. To change your address, please complete the Residency Certification/Address Change form and return it to the Payroll Office.

US Dept. of Justice INS I-9 form - All U.S. employers are responsible for completion and retention of Form I-9 for each individual they hire for employment in the United States. This includes citizens and noncitizens. This form must be completed in the Payroll Office in the Control Tower Building on or before the employee’s first day of work.  Along with the form, new hires must submit original documents to verify identity and eligibility to work in the United States.  Please contact the Payroll Office at 362-0763 with questions or to make an appointment to complete the I-9 form.

The Emergency & Municipal Services Tax (formerly the Occupation Privilege Tax) - This tax has increased to $40 per year  but the new law requires the tax be withheld in equal amounts form each pay. 

  • For faculty and staff paid monthly – the amount deducted will be $3.33 each month (Jan-Dec).
  • For staff paid biweekly – the amount deducted will be $1.53 each pay.

If you expect to earn less than $12,000 in the calendar year you can be exempted from this tax – BUT – you need to complete the Local Services Tax Exemption Certificate. The burden rests on the employee to pursue exemption.

Drug-Free Workplace Policy Notification Form  is required by the University for all employees except student workers. Review this form, acknowledge your understanding and agreement by completing all of the highlighted areas of this form.

Direct Deposit Form - Direct deposit is the most secure and efficient way for faculty, staff and student workers to receive their payment.  Funds deposited can be accessed whether or not the employee is actually on campus.

Certification of English Language Fluency for Teaching - To be completed for faculty only.

After-Hire Information Form -  This is an employee information form to be completed on a voluntary basis.
