The University provides you with a basic level of group term life insurance at no cost at one times your annual salary rounded to the next higher thousand, capped at the plan maximum of $50,000, but not less than $10,000.

Subject to certain proof of insurability requirements, you may purchase optional life insurance in amounts which are a multiple of your salary base, up to the plan maximum. You pay the full cost of optional life insurance according to an age-graded rate schedule.
